Program Guidelines

  • Patient's portion of the bill is due day of service
  • $50 reinstatement fee if your plan lapses
  • Plan is only honored at Cornerstone Dentistry. It cannot be used at any other dental office

(Parents & their children up to age 18)

  • $595 - 2 members
  • $745 - 3 members
  • $895 - 4 members
  • $110 per additional member
  • Additional $50 per family member requiring periodontal cleanings & maintenance

This program is a discount plan, NOT a dental insurance plan. It cannot be used:

  • In conjunction with another dental plan or financing program unless it is discussed with Dr. Lascari beforehand
  • For treatment which, in the sole opinion of our doctor, lies outside the realm of her capability
  • For referrals to specialists
  • For hospitalization or hospital charges of any kind
  • For costs of dental care which are covered under automobile or medical insurance
  • For services of injuries covered under worker's compensation
